The BBC was found to have committed a "major breach" of industry standards by omitting to mention that the narrator of a film about the Gaza conflict was the son of a Hamas official, as ruled by British broadcasting authority Ofcom.
An inquiry by Ofcom into the programme Gaza: How to Survive a Warzone has found that the film was "substantially inaccurate".
The head of the BBC had previously apologised, saying there had been "a serious error in relation to factual correctness".
Ofcom has instructed the BBC to air a evening statement about its conclusions.
"As this represents a serious breach of our code, we are directing the corporation to air a statement of our conclusions against it on BBC Two at 9pm, with a date to be determined," the regulator stated.
'Potential to Erode Confidence'
The authority's report declared: "The inquiry we conducted concluded that the documentary's failure to disclose that the presenter's parent held a position in the Hamas-administered administration was substantially inaccurate.
"It meant that the audience did not have vital facts which may have been extremely pertinent to their assessment of the voice and the information he presented.
"Confidence is at the core of the connection between a television network and its public, especially for a state-funded media outlet such as the BBC.
"This error had the risk to erode the exceptionally strong levels of trust that audiences would have placed in a documentary produced by the BBC about the Gaza conflict."
The programme was pulled from the online platform in the month of February after the presenter's familial connections were revealed, and in the month of July an internal BBC review determined it infringed the broadcaster's journalistic standards on truthfulness.
A corporation official stated: "The authority's determination is in line with the findings of the internal assessment, that there was a major oversight in the programme in relation to the corporation's content standards on truthfulness, which aligns with clause 2.2 of Ofcom's Broadcasting Code.
"We expressed regret for this and we accept Ofcom's decision in full. We will comply with the sanction as soon as the date and wording are finalised."
